High School Transcript, Previous Report Card, Withdrawal Grades and IEP/504 documentation

Proof of Residence (1 of the following)

• Utility Bill ( gas, water or electric) with Parent's name and address.

• Lease or mortgage statement

Birth Certificate and Custody Paperwork

Custody paperwork will be needed if parents are unmarried, divorced or separated.

Immunizations on TN Dept. of Health Form/Physical

If you are transferring from In state or county then the physical is not required.
